Fill out claim information, including the policy owner's information

Enter the policy owner's phone number to confirm the submission of completed claim application

Aviva's response period

Immediately after successfully submitting the claim request:

  • laptop You will receive a complete notification on your screen
  • phone_android Aviva's confirmation message (SMS) will be sent to the registered phone number
  • emailFor Hospitalization/Surgical benefits, Aviva will send a response letter within five business days
  • emailFor Death/Critical Illness/ TPD benefits, Aviva will send a response letter within seven business days

Note: The response period is only applicable when the claim document is fully and properly submitted. In case of additional documents, we will send a letter via the address provided on the contract
